About Evergreen Hospice’s Home Hospice Care and Elder Care
Our Mission: Providing Person and Family-Centered Care for the Mind, Body, and Spirit
at the End of Life’s Journey
At Evergreen Hospice, we are committed to delivering compassionate, personalized care that enhances the quality of life for patients and their families during one of life’s most challenging moments. With a deep understanding of the emotional and physical toll that end-of-life care can bring, our dedicated team is here to guide and support you every step of the way.
A Legacy of Competent and Caring Hospice Services
Founded on the principles of empathy, respect, and expert care, Evergreen Hospice has been serving the Kansas City community with unwavering dedication. Our mission is to provide comprehensive hospice and palliative care services that meet the diverse needs of our patients and their families. We understand that every journey is unique, which is why we tailor our care plans to address the specific challenges and wishes of each individual. Whether it’s managing symptoms of a terminal illness, providing home hospice care, or offering grief support to loved ones, we are here to ensure that no one faces this journey alone.
Our team of experienced professionals is not just skilled in medical care; they are also deeply committed to the emotional and spiritual well-being of our patients. From our hospice nurses to our dedicated volunteers, everyone at Evergreen Hospice shares a common goal: to provide a support system that wraps around both the patient and their family. We believe that end-of-life care should be about more than just managing symptoms—it should be about honoring the life that was lived and creating meaningful moments in the time that remains.
Evergreen Hospice is proud to offer services that go beyond the basics. Our Just Live! program, for example, is designed to help patients fulfill lifelong dreams and enjoy their favorite activities, even as they navigate the challenges of elder care and hospice care. Whether it’s taking a patient to a sports game or arranging a visit to a favorite place, we strive to make the impossible possible, bringing joy and fulfillment to our patients and their families.
As a non-profit organization, our focus is always on the patient, not profit. This allows us to dedicate our resources fully to providing the highest quality care. Our smaller size compared to other hospices means that we can offer more one-on-one attention, ensuring that each patient receives personalized care that meets their unique needs. We also take pride in our community-based approach, working closely with local healthcare providers, nursing homes, and assisted living facilities to ensure seamless care for our patients wherever they call home.
At Evergreen Hospice, we know that end-of-life care is about more than just medical support—it’s about creating a peaceful, dignified, and loving environment for the patient and their family. We are here to provide that support, to walk with you through every step of the journey, and to ensure that your loved one’s final days are filled with comfort, care, and compassion.

Hospice Frequently Asked Questions
Hospice is a philosophy of care that focuses on comfort, rather than a cure, for anyone with a life-limiting illness. Hospice focuses on “quality of life” issues. It focuses on caring. Hospice services are provided wherever the patient calls home whether it be a private home, assisted living apartment, or nursing home. Finally, hospice provides support to help meet the patient’s, family’s, and caregivers’ physical, emotional, and spiritual needs.
Many people with a serious illness use hospice care. A serious illness may be defined as a disease or condition with a high risk of death or one that negatively affects a person’s quality of life or ability to perform daily tasks. Examples of serious illnesses include dementia, heart failure, chronic obstructive lung disease, and cancer.
Despite the benefits of using hospice care, many people wait to receive hospice care until the final weeks or days of life. It is important to talk with your doctor about your illness and how your disease is progressing. Starting hospice early may be able to provide months of meaningful care and quality time with loved ones.
Hospice does nothing to speed up or slow down the process of dying. Hospice care manages symptoms and discomfort for the best quality of life and to minimize suffering. Choosing hospice does not mean death is imminent. Hospice care neither hastens death nor prolongs life but instead allows for better quality of life.
Medicare, Medicaid, and most private insurances pay for hospice care.
Hospice care can provide a range of different services depending on your symptoms and end of life care wishes. You will be assigned a care team consisting of the following disciplines: Nurse, Hospice Aide, Social Worker, Chaplain and Physician. Others may join your team as needed and desired, such as: a trained Volunteer, Music Therapist, Pet Therapist and Grief Support.
A Hospice registered nurse is available via telephone 24 hours a day. Hospice staff members do not stay with the patient 24 hours per day. We provide clinical, emotional and spiritual support through visits. As your condition changes we will provide more frequent visits, as well as supporting, educating, and training your caregivers.
- An interdisciplinary care team: Care is provided, and services are coordinated by an interdisciplinary team consisting of a physician, nurse, social worker, and chaplain, as well as other specialists such as pharmacists and nutritionists.
- Financial coverage: 100% covered through Medicare, medical supplies are covered, as well as durable medical equipment, i.e. hospital bed, walkers, and wheelchairs.
- We support the patient, the caregiver, and the family. We do not want anyone to walk their journey alone.
